ID Cards

When you need dental care, be sure to present your Humana DHMO ID card or your UnitedHealthcare Dental ID card (see examples below) to your provider. You cannot use your UnitedHealthcare Medical ID card for dental benefits. You should receive the card for your plan at your home address shortly after Annual Enrollment or after any changes are made in your coverage.
