ID Cards

You will need to present your newest Medical ID card to your provider (see example below). You should receive the card at your home address shortly after you enroll or make changes to your coverage. If you should need additional or replacement ID cards, you may log in to the UnitedHealthcare Web site (see Contacts at the left) to print temporary ID cards or request cards to be mailed.
