ID Cards
When you need a prescription drug, be sure to present your CVS Caremark Prescription Card to the pharmacy (see example below). You cannot use your UHC Medical ID card for prescription benefits. You should receive the card at your home address shortly after you enroll or make changes to your coverage.

If you need replacement or additional CVS Caremark Prescription Cards, follow these steps:
- To print a temporary CVS Caremark ID card, go to the CVS Caremark Web site (see Contacts at the left) and register. When the registration requests your Participant/Cardholder ID, enter your Social Security number with no dashes. After you have completed registration, click on the “Prescriptions and Coverage” tab, and select “Print ID Card.” You may present this temporary ID card to your pharmacist.
- To request that a new prescription drug ID card be mailed to you, call CVS Caremark Customer Care (see Contacts at the left).
