Health Assessments

You can get a better overall evaluation of your health status with the online Health Assessments in addition to the health screenings from Occupational Health or your personal physician. These confidential assessments identify your health risks and strengths and give you a roadmap of what you can do to make improvements. Children’s provides two opportunities for you to complete a Health Assessment. 

• You may complete the Health Assessment from Healthyroads. The Healthyroads Health Assessment is available to all Children’s employees. You do not have to be enrolled in a Children’s medical plan option. Log in to the Healthyroads Web site (see Contacts at the left) and click on “Take Your Personal Health Assessment.”
• In addition, if you are enrolled in a Children’s medical plan option, you can also complete a Health Assessment from UnitedHealthcare (see Contacts at the left). Just log in to your account and click on “Health Assessment.”
