Welcome to the Prevention and Disease Management section of My Life at Children’s. Here you will find information about support for staying healthy or managing a chronic condition. Select an option from the menu at the left for more details.
Programs for You
NurseLine Services
Reach out to a registered nurse any time, day or night. All Children’s employees and their family members are eligible to use the NurseLine services from UnitedHealthcare. You do not have to be enrolled in a medical plan option. Children’s provides this benefit to all employees at no cost to you.
NurseLine nurses are experienced professionals with an average of 15 years of experience and are a great resource to use when you need help choosing the right kind of care, managing a chronic condition, understanding different treatment options and much more.
Nurses can help you:
- Choose appropriate medical care (For example: "Do I need to go to the ER for this?").
- Find a doctor or hospital.
- Understand treatment options.
- Achieve a healthier lifestyle.
- Seek out available resources.
- Get your medication questions answered.
- Find a doctor or specialist, and check if a doctor is in-network and/or available.
- Help with booking doctor appointments.
Through NurseLine, you also have access to an audio health information library where you can pick from over 1,000 health and well-being topics. Most messages are available in Spanish.
If you are enrolled in a Children’s medical plan option, you have a choice of calling the NurseLine or chatting online with a nurse, if you prefer. Log in to the UnitedHealthcare Web site and go to the Live Nurse Chat (see Contacts at the left).
Preventive Care
If you are covered under a Children’s medical plan option, your in-network preventive care (such as annual checkups, immunizations, screenings, etc.) that is recommended based on age, gender and medical history is covered at 100% under all three medical plan options. LiveWell encourages you to take advantage of this benefit to detect and address any health issues early on. Visit www.uhcpreventivecare.com for guidelines on routine preventive care services to help maintain your health. Follow these guidelines, along with the advice of your doctor, to help you stay healthy.
Onsite Mobile Mammography
LiveWell provides a convenient "no excuses" opportunity for our female employees to get a routine screening mammogram by bringing a mobile mammography unit on site annually. While Children's does not recommend that you use this mobile mammography unit over other mammogram providers, you are encouraged to get this important screening. See the Daily Briefing or LiveWell Events for the next scheduled visit.
ActiveHealth Disease Management
You have access to ActiveHealth Disease Management programs if you are enrolled in a Children’s medical plan option. Your doctor plays an important role in managing your healthcare. You play an important role as well. ActiveHealth delivers services that give you the information you need to be an active participant in your health. Whether it's learning about an opportunity for better care, a potential medical issue or advice on managing your chronic conditions, ActiveHealth gives you the information you need to work closely with your doctors and make informed medical decisions. The services are offered to you at no charge as part of your health plan, and your health information is kept strictly confidential.
If you and your dependents have a serious or chronic condition (such as asthma, diabetes, high blood pressure or heart disease), you are eligible to participate in an ActiveHealth Disease Management Program. See the ActiveHealth Chronic Conditions list under Resources at the left to know what conditions are eligible.
ActiveHealth may call you or send you a letter asking if you want to participate in this free, voluntary and confidential program. You may also contact ActiveHealth directly (see Contacts at the left). If you choose to participate, you will work with a personal Nurse Care Manager who will:
- Review your health information with you.
- Discuss health targets and goals.
- Prepare a plan to help you meet your health goals.
- Provide information about warning signs and symptoms, and what to do if they occur.
- Suggest questions to ask your doctor.
- Identify ways to stay healthy.
- Send you helpful educational materials.
- Touch base with you about four times a year via telephone.
