Children’s is committed to helping you and your family achieve better health. We want you to feel better. To manage chronic diseases. To get more out of your life. So in 2010, we are expanding our focus on wellness by providing new tools, resources and programs to help you get and stay healthy. Here is a look at two programs.
Healthyroads Health Coaching
Get help with weight management or tobacco cessation with the support of a Healthyroads Health Coach. Participation in either of the Healthyroads programs is voluntary and free.
Weight Management
When you sign up, you can receive:
- Up to 52 sessions of weekly one-on-one telephonic coaching with a registered nurse.
- A Get Started! Rewards kit of your choice after you complete three phone calls with your Health Coach. One option includes a Healthyroads cookbook, Healthyroads nutrition bars, pedometer, exercise bands, relaxation skills kit and body tape measure.
To qualify, you (or your covered dependents) must be enrolled in a Children’s medical plan option, be 18 or older and have a body mass index (BMI) of 30 or higher (or 25 or higher with an additional health problem such as high blood pressure, diabetes or heart disease). Tobacco Cessation
When you sign up, you can receive:
- Up to 52 sessions of weekly one-on-one telephonic coaching with a registered nurse.
- Free over-the-counter nicotine replacement therapy (gum, patches, etc.).
In addition to this program, the Children’s prescription drug plan covers prescriptions such as Chantix and Zyban at the generic level.
To qualify, you (or your covered dependents) must be enrolled in a Children’s medical plan option, be 18 or older and be a tobacco user. Click on the Tobacco Cessation link to the left for more details.
Learn how you and your covered dependents can better manage serious or chronic conditions such as asthma, diabetes, high blood pressure or heart disease with ActiveHealth Disease Management.
If you and your dependents are enrolled in a Children’s medical plan option and have a serious or chronic condition, you may receive a phone call or letter from ActiveHealth inviting you to participate (after Jan. 1, 2010). The program is free, voluntary and confidential.
If you choose to participate, you will work with a personal Nurse Care Manager who will:
- Review your health information with you.
- Discuss targets and goals related to your conditions.
- Prepare a plan to help you meet your health goals.
- Suggest questions to ask your doctor.
- Give you information about warning signs and symptoms and what to do if they occur.
- Identify ways for you to stay healthy.
- Send you helpful educational materials.
- Touch base with you about four times a year via telephone.
